本剧通过描述了郁郁不得志的地下诊所医生金鑫，为筹集给小艾的聘金不得不靠非法堕胎而谋取利益，进而遭到所为上天的惩罚，迫不得已必须在规定的时间内拯救那些身上背负着罪恶而且他还非常厌恶的人，比如贪财的皮条客小刘和房东、嫌贫爱富的医学教授皮老师、贪图地位的同窗陈杰以及拜金的女友小艾等人，在拯救这些人的过程中，他逐渐幡然醒悟，终于发现原来他自己身上也背负着那些他曾讨厌的罪恶，而拯救这些人的过程，就是净化自己心灵的过程。为此当他得知嫌弃自己的女友小艾深处险境的时候，奋不顾身地想尽一切办法来拯救小艾的故事，阐述了救赎他人，即是救赎自己的主题思想。。山田日後承認在開拍此片之前，已心諳渥美清的肺病步入末期，決定此章為最後一章，並安排由淺丘瑠璃子演的莉莉送寅次郎踏上人生的最後一程。分別15年後，兩人於如詩如畫的奄美大島重逢，感情依舊，鬥氣繼續，沒完沒了似的。正如山田所想，雖然系列必須完結，但始終不想為寅次郎的故事／人生劃上句號。結局如常開放，代表寅一直繼續他的旅程，日本人依然有夢。 公映後翌年，渥美清病逝，享年68歲，本片亦成為他的遺作。 Yamada knew before production began that Atsumi's health has deteriorated to such an extent that Tora-san to the Rescue would be the last film in the series.
